Insurance

We accept most insurance carriers. Please call your health insurance in advance to verify we are a provider for your insurance carrier. Our contracts with insurance carriers require us to collect the appropriate co-pay and/or deductible at the time of service. If we are not a provider for your insurance, you will be responsible for the entire charge at the time of service.
Please have your insurance card or Medicaid card with you at the time of your visit.
For those patients that are Self Pay, we can offer a Self-Pay Discount if payment in full is made at the time of the visit. No discount will be given if we must bill you. We are happy to discuss payment plan options with you.
Please note that we accept Mastercard, Visa, Discover, and American Express credit cards, as well as bank debit cards.
